• Directive containing the senate decisions regarding the course

• Co-Op internship application petition

• Internship agreement to be signed with the institution where the internship will take place

• Project proposal to be completed at the institution where the internship will take place (to be submitted to the department)

• Student evaluation form to be filled out by the institution where the internship is carried out. This document must be completed by the authorized person at the institution and delivered to the student in a sealed envelope.
